Transitional Care
The transitional care program at Northwest Health - Starke, sometimes called a "swing bed" program, allows patients to rehabilitate in a safe, skilled nursing environment before returning home from a hospital stay. Patients may qualify for the program, with a physician's referral, and extend their recovery following an acute care, three-night qualifying hospitalization.
This short-term rehabilitation and skilled nursing care is for patients who are not ready to return home after being in the hospital. The program is designed for those who need more time to strengthen and heal before they can step back into the routine of daily living.
Service include:
- A safe, comfortable hospital environment with private rooms
- Physicians and registered nurses on duty around the clock
- Administration of IV fluids and medications
- Rehabilitation services, including physical, occupational and speech therapies
- On-site lab, radiology and pharmacy
- Respiratory therapy
- Skin and wound care
- Individualized care plan, care coordination and discharge planning by care managers
- The ability to transfer from any hospital into the program
